ON
TRACK
ARE
YOU A YOUNG PERSON WHO WILL BE LIVING AT ON TRACK?
On
Track is a
place where some young people (both males and females) who are aged
between 15-18 can live for some time to continue with their education
and training and to prepare for living independently. On Track
is
located at Roselands, a short walk from Punchbowl Station.
As
a resident at On Track, here is what you can expect:
- You can continue to
live
at On Track until you turn 18 or until you are ready to move out to
some other type of accommodation that suits you.
- You will be living with
up to five other young people, both males and females
- There are Youth Workers
there to help you during the day and a worker sleeps over every night.
- You will learn how to
look after yourself as an adult for when you move out.
- You will have your own
room and own furniture and share other living areas.
- You will need to attend
a
Weekly House Meeting with other residents to plan how you will share
the household chores including cooking
- You need to be
attending school or other training, be working or be looking for work
- You will need to pay
board
if you have an income or receive a benefit. This helps to cover the
costs of food and accommodation and helps you to learn budgeting skills
- You will be helped to
develop other skills such as cooking, shopping, caring for your
clothes, caring for your health, preparing your resume for
work,
etc
- You will be helped to
follow
your hobbies or interests. This could be things like playing a
sport, learning to play a musical instrument, dancing, fitness, or
other suitable activity you wish to pursue.
- When you are ready to
move
on, we will help you find the best place to live and help you to set
yourself up with furniture and other items. We also offer aftercare
support until you turn 25.
When
you go to visit ON TRACK with your worker you will be given a copy of
the Information for On Track Resident's booklet,
which will tell you all about how things work in this
program.
You will also be able to ask questions and check out the place to make
sure you are happy to be living there before you move in.

help you to determine if the young person is eligible for referral to
Stretch-A-Family’s On Track program.
Program Aim
- To prepare young people
for independent living.
- To support young people
to
develop skills to enable them to make informed choices about their
future accommodation, career, educational, recreational and general
life options.
Please
Note
Stretch-A-Family’s On Track
program is not a crisis accommodation service. Young people
in
crisis may be referred but the referring person will need to make
temporary accommodation provisions for the young person pending
assessment.
Length
of Stay
Long Term: up to 4 years.
Number
& Age of Young People
Up to 6 young people aged from 15 to 18 years.
Basic
Assessment Criteria
- Assessed as ready to
work towards independent living.
- Suitable for the
supervision level provided by the service.
- Attending school,
training, work or actively seeking work.
- No history of excessive
violence or endangering behaviour
- No current alcohol or
drug dependencies
Referrals
Referrals are taken by the Caseworker (02) 9758 9378. Suitable
referrals must then be approved by the Department of Community Services
Metro Central Region. The Assessment includes a two week
trial to
make sure that the program has the capacity to meet the needs of the
referred young person.
